The Position:
As a member of an interdisciplinary team, which includes physicians, nurses, service coordinators, and PT/OT, the Transplant Social Worker is responsible for assessment and intervention with any patient or family along the health care continuum where psychosocial and/or high risk factors are a part of or interfere with the patient's treatment, health outcome and discharge plan. The Transplant Social Worker role includes (1) outpatient work including assessments, annual visits and ongoing management of pre- and post-transplant patients; and (2) discharge planning/inpatient work as lea! d inpatient social worker for new transplants and all living d! onors when in the hospital. The Transplant Social Worker is a member of the Fletcher Allen Health Care transplant team, the Fletcher Allen Health Care Case Management & Social Work Department, and the Case Management Social Work Department Renal Social Work Group.

The Candidate:
?MSW from an accredited school of social work is required. Licensure through the State of Vermont is encouraged.
Experience working in a human service organization, a health care setting, or a managed care system is required. Two or more years’ experience as a transplant, renal or medical social worker is highly preferred.
