We are a Level I Adult and Pediatric Trauma Center serving a population of one million throughout Vermont and Northern New York. With almost 7,000 employees, we are the largest private employer in the state of Vermont.The Position: As a member of an interdisciplinary team, which includes physicians, nurses, service coordinators, and PT/OT, the Transplant Social Worker is responsible for assessment and intervention with any patient or family along the health care continuum where psychosocial and/or high risk factors are a part of or interfere with the patient's treatment, health outcome and discharge plan.
The Transplant Social Worker role includes (1) outpatient work including assessments, annual visits and ongoing management of pre- and post-transplant patients; and (2) discharge planning/inpatient work a! s lead inpatient social worker for new transplants and all liv! ing donors when in the hospital. The Transplant Social Worker is a member of the Fletcher Allen Health Care transplant team, the Fletcher Allen Health Care Case Management & Social Work Department, and the Case Management Social Work Department Renal Social Work Group.The Organization: As an academic medical center dedicated to being in service to the patient, community and medicine, Fletcher Allen (www.fletcherallen.org) seeks to improve the health of the people in the communities it serves by integrating patient care, education and research in a caring environment.
