PharmPro is seeking a temporary (long term contract) Staff Clinical Pharmacist ASAP , hours M-Friday: 8:00am-5:30pm, no weekends or holidays. Must be Vermont Licensed in good standing.
POSITION OVERVIEW
Clinical Pharmacy:
- Monitors drug therapy of patients for appropriateness of drug and dose, drug interactions, contraindications, and adverse reactions
- Provides pharmacokinetic consults and renal drug dosing adjustments per hospital protocols
- Performs IV-to-PO automatic dosing changes and therapeutic substitutions
- Provides clinical consultation and clarification to practitioners
- Participates in clinical “rounds†with nursing and medical staffs and clinical collaboration meeting
- Participates in drug use evaluations and evaluation of new drugs for! formulary addition
- Demonstrates the knowledge and sk! ill necessary to identify and meet provide pharmaceutical care to the population served, including neonatal, pediatric, adult, geriatric, acute care, obstetric, chemically dependent, and rehabilitation patients.
