Description: ADDISON COUNTY HOME HEALTH & HOSPICE is looking of Physical Therapist - PT right now, this job will be ordered in Vermont. Do you want to provide Physical Therapy services to agency patients in a community setting?
We are looking for the right PT with the sk! ills to provide quality treatment to agency patients. The plan of care is developed by the therapist and approved by the physician. Our therapists use independent judgment based on common principles of pathophysiology and accepted standards of care.
Pediatric experience preferred, but not required
Full or Part - time
QUALIFICATIONS:
Graduate of an Physical therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association; and is eligible for or has taken and passed the National Registration Examination of the American Physical Therapy Association.
Works collaboratively with patients, families, colleagues, and other community agencies.
Must be able to adapt to patients status and needs. Occasional schedule changes due to patient condition, weather and environmental conditions and/or new patients.
Home visits daily.
Licensure: Physical Therapist - State of Vermont.
