Diabetes Educator

Immediate opening for a full-time Diabetes Educator to serve as the clinician who regularly works with outpatient clinic patients on the education, coaching, and related support for the patient’s diabetic care. Will provide: ed! ucation on diabetes and the disease process to diabetics, families and individuals; education and instruction to diabetics on medication, diet and nutritional monitoring, foot care, disease complication prevention, blood glucose monitoring and injections. With diabetic patient and/or patient’s caregiver, will monitor blood glucose, medication and insulin control. Will assess, monitor and implement treatment plans for patients with focus on prevention and health improvement. Will document assessments, interventions and plans including recommendations and goals, progress notes and discharge summaries. Will perform other duties and support to co-workers/office needs as required. Will provide coordination of diabetic patient care with providers, members of the Community Health Team and other agencies/groups. Accepts and works with all patients referred and scheduled by providers.
Requirements:
- Bachelor’s of Science, Nursing required
- Adv! anced Practice degree preferred
- Certified Diabetic Ed! ucator required
- Electronic Medical Records proficient
- Interpersonal/political savvy in interdisciplinary and provider relations
- Experienced with outpatient/clinic work environment and protocols
- Work independently and autonomously
- Two years minimum experience as diabetes educator in a clinic setting.

Grace Cottage Hospital is a Critical Access hospital composed of 19 beds and 167 employee! s. The service area includes Windham County in Southeastern Vermont. An area representing a population of approximately 26,000. The facility provides inpatient care; full array of radiology support; rehabilitation (PT, OT and Speech), outpatient clinic. Our member medical staff provides services such as Family Health, Pediatrics, Mental Health, Podiatry, and Emergency Services. The hospital utilizes Cerner EMR software.

Townshend was chartered in 1753. While the early settlers focused on farming, in the late 1700’s opportunities in manufacturing, retail and hospitality grew. Today Townshend maintains a quiet idyllic look typical of a quaint New England village. Townshend is nestled in the upper West River Valley just 17 miles north of Brattleboro. It is a small community of about 1,200 full time residents and roughly 469 households.
Townsend is located 15 minutes to the interstate for easy access to cities like Boston and Burlington in abou! t 3 hours.
