Description: Fletcher Allen Health Care is in need of Social Worker II - Outpatient - Part Time right now, this position will be situated in Vermont. As a member of an interdisciplinary team, which includes physicians, nurses, service coordinators, PT/OT, the Social Worker II is responsible for assessment a! nd intervention with any patient or family along the health care continuum where psychosocial and/or high risk factors are a part of or interfere with the patient's treatment, health outcome and discharge plan. The Social Worker II utilizes advocacy skills, intervenes therapeutically supportively, provides resource and referral information, offers financial guidance and assistance, and assists with discharge and post discharge planning. The Social Worker II serves as a liaison to health and social agencies in the community.
This position works with the Children's Specialty Clinic (and at times with children and families hospitalized in Pediatrics). It serves on the endocrine, pulmonary and renal clinics.
This role is for one day/week, currently Thursdays. It could be combined with other part-time positions in the department.
Education:
MSW from an accredited school of social work. Licensure through the State of Vermont is encouraged.
Experience:
Experience working in a human se! rvice organization, a health care setting, or a managed care system is required.
