Physical Therapist - Home Health - (PT - HH)

StartDate: ASAP Pay Rate: 47.50 *

H! ome healthcare company is seeking a self starting, highly motivated Physical Therapist to join the team. Therapist will treat an Adult/Geriatric population seeing approximately 5 patients per day. Candidates must be willing to support a friendly, positive and professional environment.

Additional Requirements/Information:

• Interested candidates must have 1 year of experience (if applicable)
• Home care experience preferred, but not required
• Candidates must be currently licensed or eligible for licensure as a PT and/or registered as a Physical Therapist per state requirements.

A physical therapist (PT) is responsible for diagnosing and treating patients of all ages who suffer from physical conditions that limit the ability to move and/or perform daily functional activities. A PT has a number of job functions, including assessing the patient and creating an individualized treatment plan to work on the areas that could use ! improvement. In assisting with a patient’s physical therapy,! a PT also incorporates preventative measures against the loss of motor skills and promotes overall healthier lifestyles. The settings worked in include:

Acute

Home health

Long-term care

Outpatient

Pediatric

Psychiatric

Rehabilitation

School

Sports medicine

Required Qualifications

• VT & NH licensure
• Homecare experience

Preferred Qualifications

• McKesson experience
