BASIC FUNCTION

To provide pediatric high tech care to infants and children in a home care setting, under minimal clinical supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care. There is often a need to work evenings, weekends or holidays.

CHARACTERISTIC DUTIES & RESPONSIBILITIES

  • Provide direct pediatric high tech nursing care to patients under minimal personal and procedural clinical supervision, following an established nursing plan of care, including: use and care of feeding tubes and feeding pumps, administration of medications, assisting with general exercise and specific physical therapy regimens.
  • Assist in personal care of patients: range of motion, bathing, oral hygiene, skin care, dressing, toileting/diapering, transfers via mechanical lifts, and other activities of daily living.
  • Observe and report changes in patients’ condition to case manager.
  • Confer regularly with case manager regarding patientsâ! €™ status and treatment plan.
  • Prepare all required clinical documentation related to assigned activities, adhering to all relevant legal regulations, technical requirements of government agencies, internal procedures, and professional standards.
  • In carrying out all clinical duties, adhere to established protocols, guidelines and standards for LNA care, consulting with senior clinical staff regarding unusual situations.
  • Complete 12 hours of CEUs yearly.
  • Other duties as assigned by supervisor.
SUPERVISION RECEIVED

General administrative and moderate to minimal clinical supervision is received from an RN case manager, Clinical Team Leader or other clinical supervisor; following Agency, State and Federal guidelines.

QUALIFICATIONS

  • Active State of Vermont License (RN, LPN, LNA)
  • Previous experience working with special needs pediatric population desirable.
  • Ability to communicate a! nd interact effectively with individuals and groups.
  • Ability to identify emergency situations and act appropriately to provide assistance.
  • Ability to travel extensively in order to conduct a home visit.
  • Physical ability to perform all personal care activities and other essential job duties per current agency standards.
  • Ability to complete additional training appropriate to the job prior to assignment.
  • Possession of a current driver’s license.
  • Automobile Bodily Injury Liability insurance with coverage amounts of $1000, 000 per person, $300,000 per accident.
