BASIC FUNCTION
To provide pediatric high tech care to infants and children in a home care setting, under minimal clinica! l supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care. There is often a need to work evenings, weekends or holidays.
CHARACTERISTIC DUTIES & RESPONSIBILITIES
- Provide direct pediatric high tech nursing care to patients under minimal personal and procedural clinical supervision, following an established nursing plan of care, including: use and care of feeding tubes and feeding pumps, administration of medications, assisting with general exercise and specific physical therapy regimens.
- Assist in personal care of patients: range of motion, bathing, oral hygiene, skin care, dressing, toileting/diapering, transfers via mechanical lifts, and other activities of daily living.
- Observe and report changes in patients’ condition to case manager.
- Confer regularly with case manager regarding patients’ status and treatment plan.
- Prepare all required clinical documentat! ion related to assigned activities, adhering to all relevant l! egal regulations, technical requirements of government agencies, internal procedures, and professional standards.
- In carrying out all clinical duties, adhere to established protocols, guidelines and standards for LNA care, consulting with senior clinical staff regarding unusual situations.
- Complete 12 hours of CEUs yearly.
- Other duties as assigned by supervisor.
General administrative and moderate to minimal clinical supervision is received from an RN case manager, Clinical Team Leader or other clinical supervisor; following Agency, State and Federal guidelines.
QUALIFICATIONS
- Active State of Vermont License (RN, LPN, LNA)
- Previous experience working with special needs pediatric population desirable.
- Ability to communicate and interact effectively with individuals and groups.
- Ability to identify emergency situations and act appropriately to provide assis! tance.
- Ability to travel extensively in order to conduct a home visit.
- Physical ability to perform all personal care activities and other essential job duties per current agency standards.
- Ability to complete additional training appropriate to the job prior to assignment.
- Possession of a current driver’s license.
- Automobile Bodily Injury Liability insurance with coverage amounts of $1000, 000 per person, $300,000 per accident.
