We are a Level I Adult and Pediatric Trauma Center serving a population of one million throughout Vermont and Northern New York. As a member of an interdisciplinary team which includes physicians, physician assistants and nurse practitioners, the Primary Care Behavioral Health Clinician (PCBHC) is responsible for providing consultation, brief cognitive behavioral assessment and intervention to a primary care population including patients with acute behavioral needs, patients who have an acute psychological response to a medical event and to patients with emerging medical or chronic medical conditions.
In addition, the clinician will provide care management and triage services for patients who need to be or are involved with the community health team, as well as work with outside behavioral health clinicians to arrange services for practice patients who need more than primary behavioral health care services. The process of care will often feature a clinical handoff from physician to clinician in which the clinician may well be interrupted during a session to meet a new patient. This is an evidence-based, population based model of care. Clinicians will be expected to develop a subspecialty in a co-morbid medical condition such as diabetes or headache and to participate in clinical research.
